dedicated Nurse Navigator Women and Children like you to be a part
of our team. Job Summary and Qualifications As the Nurse Navigator,
you function as a member of the multidisciplinary team and serve as
an advocate and educator for patients with all types of medical
conditions. The Nurse Navigator’s primary function is to support
patients through complex healthcare environments for the
optimization of care and outcomes. The Nurse Navigator facilitates
patient follow-up through coaching, information transfer, and
communication in order to improve continuity of care, enhance the
medical management of complex diseases, and streamline the
intervention process. The Nurse Navigator guides patients and
families through the healthcare system by assisting with access
issues in an effort to expediate timeliness of care and treatment.
Navigators achieve this goal by building relationships with
patients and physicians, coordinating the defined plan of care,
assisting with appointments, assisting to resolve barriers,
providing education, and connecting with community resources. The
navigator also assumes responsibility and accountability for the
management of resources to achieve efficient, high-quality outcomes
for patients, including support with interdisciplinary and
cross-facility coverage and collaboration. The navigator is
responsible for timely data collection that promotes tracking of
the navigation program performance in line with HCA Healthcare,
division and facility goals (growth, quality, practice guidelines,
etc). The navigator identifies opportunities to streamline care
practice through evidence-based guidelines and ensures quality,
patient-focused care. EDUCATION - Bachelor’s Degree preferred
EXPERIENCE / CERTIFICATIONS - 3 years of clinical experience –
Required - 3 years of experience in as a Nurse Navigator and/or
patient educator in service line – Preferred - 3 years minimum
Peds/PICU experience - Preferred - Basic Cardiac Life Support
(preferred if remote position) – Required - Registered Nurse -
Currently licensed as a registered professional nurse in the state
in which he or she practices, in accordance with law and
regulation. – Required - Service Line Specialty Certification –
Preferred - 3 plus years minimum Peds/PICU or CICU- Preferred.
