Job Description:
Vital Caring Group is seeking a dedicated and skilled Physical
Therapist to join our team of healthcare professionals, bringing a
personalized approach to comprehensive home health solutions. In
this role, you will collaborate with interdisciplinary teams to
plan and administer medically prescribed physical therapy
treatments for patients of various ages, helping them improve
mobility, relieve pain, and prevent or limit physical disabilities.
This position requires a compassionate professional dedicated to
providing patient-centered care, enhancing patient outcomes and
quality of life in the comfort of their homes. Evaluate patient's
medical history to develop tailored treatment plans. Assess
patient's physical abilities and limitations to establish realistic
goals. Administer hands-on therapy and prescribe physical exercises
to enhance patients' health. Monitor patients' progress and modify
treatment plans as needed. Educate patients and families about
health programs and maintenance. Maintain patient records and
ensure compliance with healthcare regulations. Strong interpersonal
and communication skills. Proficiency with physical therapy tools
and techniques. Critical thinking and problem-solving abilities.
